AB458-AA1,1,11 At the locations indicated, amend the bill as follows:

AB458-AA1,1,3 21. Page 3, line 5: delete the material beginning with "by a" and ending with
3"this state" on line 6.

AB458-AA1,1,4 42. Page 3, line 7: delete "even".

AB458-AA1,1,5 53. Page 3, line 7: delete "physician providing" and substitute "provider of".

AB458-AA1,1,7 64. Page 3, line 7: delete the material beginning with "is located" and ending
7with "the state." on line 8 and substitute "satisfies all of the following criteria:".

AB458-AA1,1,8 85. Page 3, line 8: after that line insert:

AB458-AA1,1,9 9"a. The provider is a certified provider of Medical Assistance in this state.

AB458-AA1,1,1110 b. The provider is currently licensed, certified, or registered in his or her
11profession in this state.

1c. The provider is located in the United States. The provider is not required to
