AB305-AA3,1,2 21. Page 4, line 21: delete "person" and substitute "individual".

AB305-AA3,1,3 32. Page 4, line 23: delete lines 23 and 24.

AB305-AA3,1,6 43. Page 12, line 5: delete "Unless any of the following is" and substitute "If any
5of the following has a direct professional relationship with or provides professional
6services directly to the declarant and is not".

AB305-AA3,1,7 74. Page 12, line 15: delete that line.
